There was no circle tab to select for “other” on question 6, so I chose “restaurant website” & left comments in “other” box. Interesting survey. Thank you.

You left out the breakpoints about whether or not wine can be brought in, whether or not the corkage will be a deciding factor? A reasonable corkage often enhance the dining experience.

I like to share my dining expereinces and often write my own review of a restaurant on urbanspoon, yelp and trip advisor. I am always on the hunt for great tasting food to re-create for my blog and cookbook!

Another deciding factor for me that you might consider adding is customer service .It is very important food can be grear and afordable but if service is not good I wont return.
